Leveraging a substantial number of deep sequencing metagenomes, our study shows striking similarities in gut microbial patterns at both taxonomic and selected gene marker levels between yCRC and old-onset CRC.This is my first research paper following my PhD training with Prof. Michael Inouye and Prof. Kathryn Holt in Melbourne and Cambridge.My research journey with CRC microbiome traces back to 2012 when I collaborated with Prof. Jun Yu and found bacterial taxa and functional genes associated with CRC. Since then, the field has progressed fast, with well-established associations between CRC and gut microbiome. However, few studies have delved into the gut microbiome concerning yCRC (aged &lt;50), partly due to its lower incidence rate compared to old-onset CRC (oCRC, aged ≥50).The gut microbiome is known to vary with age. With phenotypic differences in disease stage and tumor location between yCRC and oCRC, one could hypothesize that the gut microbiome might also differ. Aiming to characterize the gut microbiome of yCRC, I collaborated with Prof. Pei-Rong Ding at Sun Yat-Sen University Cancer Center since 2021.Negative findingsIn our study we conducted deep metagenomic sequencing on stool samples collected from a sizable cohort of CRC patients, including 167 yCRC. By comparing microbiome compositions between yCRC and oCRC, we observed only a few bacterial taxa that met the statistically significant threshold. Notably, the associations between age and microbial features across all studied samples were weak. Despite our rigorous efforts involving various analytical strategies, such as comparing individuals aged &lt;40 to those aged &gt;60, our investigations failed to yield promising results.The Fudan StudyWhile we were disappointed, a study by Yang et al. from Fudan University working on the same topic was published. They reported distinct signatures of yCRC relative to oCRC. We got even more disappointed. After thoroughly reviewing the Fudan paper multiple times, I found that their findings on oCRC weren't corroborated by other studies. One possible explanation is that their results primarily relied on 16S rRNA sequencing, although they were validated by shotgun metagenomic sequencing. Fortunately, the Fudan team also made the shotgun metagenomic sequencing (raw) data publicly available for 200 individuals. I downloaded and analyzed this data using our pipeline. Surprisingly, my findings diverged from those reported in the original paper. Notably, I found several well-known CRC-associated bacteria (Clostridium symbiosum, Hungatella hathewyi, Peptostreptococcus stomatis and Parvimonas micra) that were enriched in both oCRC and yCRC. Encouraged by these promising findings, we integrated our data with the Fudan one, along with other published datasets, ultimately uncovering consistent signatures for both oCRC and yCRC.Editor's SupportThen we found ourselves in a challenging position: eager to publish our results but needing more confidence in presenting contrasting findings to a previously published study. Despite our reservations, we decided to take a chance. I reached out to Dr. Javier Martinez-Vesga to discuss our intention to write a short paper detailing our contrasting results to a study published in Nature Communications. Interestingly, Javier suggested that we expand our scope and submit a full research article instead, highlighting the new data we had generated from numerous CRC patients.Reviewers' SupportDuring the peer-review process, we received positive feedback from all reviewers, who recommended conducting a more in-depth analysis of the bacterial genomes and exploring bacterial toxin and virulence factors. Following their suggestions, we conducted a more focused analysis, leading us to uncover insights into well-known CRC-associated taxa with clear pathogenic pathways: Fusobacterium nucleatum, Bacteroides fragilis, and Escherichia coli.Story of F. nucleatumIn our studied samples, we detected both F. nucleatum (Fn) and its close relative F. animalis (Fa; also known as Fn subspecies animalis, Fna). Strikingly, Fa had higher abundance and prevalence compared to Fn. This observation coincided with a very recent study published in Nature, which reported the enrichment of a distinct Fa clade in CRC niche, consistent with an earlier study. Moreover, in our Chinese cohort, we found no differentiation in the presence of the fadA gene (encoded adhesion protein A, which mediates Fn attachment to and invasion of host epithelial cells) between samples enriched with either Fn or Fa.Story of B. fragilisWe identified two distinct clusters of B. fragilis in our data. In line with a previous study, each individual was dominated by one strain. Pathogenic B. fragilis strains can produce enterotoxin, which plays a role in cell cycle regulation. While the specific identities of these two strain clusters were unknown to us, we observed differential abundance and prevalence of the enterotoxin-encoding gene but between the two clusters.Story of E. coliOnly one strain cluster of E. coli was identified in our samples. As a commensal bacterium in the gut, most E. coli strains are non-pathogenic. However, pathogenic strains often harbor the pks genomic island, which encodes colibactin, a compound implicated in DNA damage. Subsequently, we examined the relationship between the abundance of E. coli and pks. Interestingly, we observed a positive association in the Guangzhou cohort, but no correlation was evident in the Fudan cohort. This population heterogeneity implies a complex microbiome background of CRC across different geographic regions.Potential impactThe shared microbiome signatures between yCRC and oCRC could serve as a foundation for developing microbiome-based diagnostic methods for CRC screening. Our study showed that microbiome-based classifiers trained for oCRC had similar prediction performance for yCRC, and viceversa. The consistency in signatures across CRC cases of different ages suggests that the microbiota may play a pivotal role in CRC carcinogenesis.Ongoing workOne of the most intriguing questions is whether these microbial signatures correlate with CRC clinical outcomes. To address this, we are actively collecting follow-up data for many of the patients included in our study and investigating the relationship between gut microbiome and CRC outcomes. Since many CRC-associated bacteria are commonly found in the oral cavity, we have also generated matched metagenomes from saliva/tongue samples for most of our patients. It's worth noting that our current study only focused on bacterial taxa. However, considering that fungal and viral signatures have also been reported to be associated with CRC, conducting further analyses on fungi and viruses will offer new biological insights into CRC pathogenesis.Simultaneously, BGI Genomics is embarking on a mission to sequence 10 million microbiome samples as part of a large-scale cancer screening project for CRC. This endeavor will yield an unprecedented volume of microbiome data for humans. <img src="https://cdn.it.bgi.com/source/website+preview+crc.jpg" alt="website+preview+crc.jpg" data-href="" style="">Over recent decades, the incidence of colorectal cancer (CRC) in individuals under 50 years old, known as early-onset or young-onset CRC (YO-CRC), has been rising according to the <a href="https://acsjournals.onlinelibrary.wiley.com/doi/epdf/10.3322/caac.21834" target="_blank">American Cancer Society</a>. Projections published on <a href="https://www.nature.com/articles/s41572-023-00432-7" target="_blank">Nature</a> suggest that by 2030, YO-CRC will constitute 11% of colon cancers and 23% of rectal cancers, with notable increases in the United States, Canada, and Australia.The diagnosis of colorectal cancer (CRC) among young adults aged between 19-39 was more likely to be dismissed or misdiagnosed with other diseases, such as inflammatory bowel disease. When a young patient is diagnosed with CRC, it is often at stage III or IV, which dramatically increases the cost and difficulties of treatment and potentially raises the risk of mortality.<img src="https://cdn.it.bgi.com/source/colotect+resized.jpg" alt="colotect+resized.jpg" data-href="" style="width: 100%;">Early screening and regular testing can be an effective way to prevent and treat CRC at an early stage. <a href="https://colotectglobal.com/" target="_blank">COLOTECT&trade;</a>, a non-invasive test that requires only a stool sample, is available for colorectal cancer screening and can provide medical professionals with evidence to support their diagnosis. This group has unique clinical and pathological traits. They differ from those seen in people aged 40-49. These findings suggest that the age threshold for colorectal cancer screening may change over time. In the United States, routine CRC screening currently begins at age 45, but as trends evolve, especially in the 45–50 age group, this starting age may be reconsidered to better address youth-onset CRC (YO-CRC). The increase in the incidence of colorectal cancer among young people raises the alarm for regular screening for colorectal health, even at younger ages. In 2016, then-28-year-old Nguyen, who was studying to become a physician assistant, suggested that her symptoms might indicate cancer. She added, “A little bit inside of me also just said, ‘Oh, your symptoms are nothing, Shauna.’” Often, patients are led to believe that a doctor's medical diagnosis is indisputable and should not be questioned, which can lead to the dismissal of early symptoms of CRC as overreactions.Symptomatic or NotMany believe that tumors or cancers only develop with symptoms, leading many patients to visit doctors only when feeling unwell. Having lost her father to colorectal cancer, she was committed to maintaining her health. Adhering strictly to a plant-based diet, exercising five or six times a week, training for a half marathon, and teaching yoga in her community, she did everything by the book. Despite her efforts, she faced a dire diagnosis.The colonoscopy revealed she was at stage III of cancer, with her colon nearly obstructed by a tumor. Kathryn underwent surgery to remove the tumor and had a temporary ostomy—a surgical opening in the abdomen to allow waste to exit into an external bag. Fortunately, by the end of 2023, Kathryn had recovered well enough to resume her passion for yoga and running.From Awareness to Prevention The increasing incidence rate of CRC and the difficulties in diagnosing it among young people are raising the alarm for early and regular screenings for colorectal health. Although it's not yet clear why colorectal cancer is on the rise among younger individuals, experts are actively investigating potential factors, including diet, obesity, physical inactivity, smoking, alcohol consumption, and family history. Newborn genetic screening (NBGS) seeks to assist in the early identification of newborns at risk for severe genetic disorders. The integration of genomics into newborn screening represents a significant advancement, enhancing disease detection while reducing false positive rates. However, sharing results about genetic carriers is a delicate balance between sharing important information and not causing unnecessary worry for families.To navigate these complex considerations, a joint study led by Nanjing Women and Children’s Healthcare Hospital and Jing-Yu Zhao at BGI Genomics, recently published in the <a href="https://jogh.org/2024/jogh-14-04044#R7" target="_blank">Journal of Global Health</a>, surveyed parents of newborns identified as carriers through genetic screening. This research aimed to gather their perspectives and inform the development of these vital screening programs.<h4 style="text-align: justify;">Research Methodology</h4>The study focused on parents of newborns identified as carriers through NBGS at the Nanjing Women and Children's Healthcare Hospital. A comprehensive survey comprising 26 questions was distributed, collecting responses from 2,390 participants. The survey delved into various aspects, including demographic information, understanding of genetic diseases, and the perceived impact of NBGS results.<img src="https://cdn.it.bgi.com/source/2398.png" alt="2398.png" data-href="" style="width: 100%;">Demographic characteristics of respondents<h4 style="text-align: justify;">Key Findings</h4>The average age of the respondents is 30.7 years, with a predominance of positive perceptions towards NBGS. Specifically:<ol><li style="text-align: justify;">Parental Knowledge and Attitude Towards NBGS: A considerable majority of parents demonstrate an admirable level of awareness regarding NBGS, acknowledging its essential role in the early detection of diseases. A significant 73.18% of the participants are of the opinion that NBGS will positively affect their future by enhancing the understanding of their child’s health conditions.</li><li style="text-align: justify;">The Dilemma of Reporting Carrier Status: Despite the potential for family anxiety, an overwhelming 98.95% of parents expressed a preference for being informed about carrier results. This indicates a strong parental desire for comprehensive genetic information regarding their children, underlining the necessity for transparency within NBGS processes.</li><li style="text-align: justify;">Impact of Prenatal Carrier Screening: The study highlights a critical observation that families subjected to prenatal carrier screening possess a superior comprehension of NBGS. This suggests that such screenings are instrumental in promoting the implementation of NBGS. Additionally, NBGS carrier reports are seen to potentially encourage parents to pursue carrier screening before subsequent pregnancies, particularly for conditions with higher incidence rates. The revelation that their baby is a carrier prompted 98.83% of couples to consider prenatal counselling before planning another pregnancy, and 91.63% resolved to inform their child about their carrier status upon reaching adulthood, notably among high-income families and those previously screened.</li><li style="text-align: justify;">Socio-Economic and Educational Influences: The study sheds light on the socio-economic and educational variances in the awareness and attitudes towards NBGS. It was observed that higher education levels and incomes correlate with an improved understanding, indicating the impact of socio-economic factors on health literacy.</li></ol><img src="https://cdn.it.bgi.com/source/3606.png" alt="3606.png" data-href="" style="width: 100%;">Impact of being a carrier on the family<h4 style="text-align: justify;">Conclusion</h4>These findings show if people are more aware of and take part in prenatal carrier screening, it could greatly improve implementation and acceptance of NBGS. NBGS also has the potential to advance carrier screening practices. Wang Leilei: Inclusion is beyond gender equalityWang, a mother of two and the first blind model in China, has overcome challenges due to being diagnosed with retinitis pigmentosa (RP) at the age of 10, a rare disease affects 1 in 5000 individuals worldwide. Her journey with RP led to night blindness, light phobia, reduced vision, and finally, complete blindness, with no specific treatment available. Despite this, Leilei continued to pursue her passion for modeling, using her feet to measure the stage during rehearsals as her eyesight deteriorated.<img src="https://cdn.it.bgi.com/source/2039.png" alt="2039.png" data-href="" style=""/>In addition to her modeling career, Wang is also the founder of Hualei Disability Service Center. She began providing modeling training to help these children improve their physical functions, develop self-confidence, and build social awareness.
